 Refractory pigmentation can be more complex than it appears: beneath the surface, it's a multifactorial process involving not only epidermal and dermal pigment but also a vascular component. That's why it calls for assessing the underlying pathophysiology first, rather than reaching for a single device. At Candela's VIP Exclusive Symposium 'Turning Back the Skin Clock,' held in Bangkok during IMCAS Asia 2026, we sat down with Thailand's Dr. Wichai Hongcharu — a KOL who has worked with Candela's picosecond laser 'PicoWay' for nearly a decade and trained at the Wellman Center for Photomedicine at Harvard. His take is measured: PicoWay's short pulse durations and range of wavelengths and handpieces make it useful across a broad range of skin types, from fair to deeply pigmented — but the more refractory the pigmentation, the more it often calls for a combined approach, integrating, where appropriate, modalities such as tranexamic acid, vascular lasers, and microneedling rather than the picosecond laser alone.
